  • You can make a referral or enquiry through our form.
  • Your request will be received by our service coordinator who will get back to you within two working days to let you know whether your case has been accepted by the relevant consultant.
  • Once your case has been accepted, you will be sent your login details to Neurorehab Hub’s Patient Portal, where you can access your appointments, payments, and meeting links.
  • Here, you will be able to submit your pre-consultation form which we will need before your first appointment.

NHS Right to Choose

Neurorehab Hub works within a clear risk assessment framework and risk identification criteria. Please refer to the guidance below to assist all decisions of whether it is appropriate to make a referral to us.

Intentional or Unintentional Self Harm

If known intent, attempts or acts taken within the past 12 weeks, we are not able to accept a referral. Please instead refer to appropriate local mental health services.

Risk From Others

If known or suspected risks of harm to the patient from others are present, we are unable to accept a referral. Please instead refer to appropriate local social support or safeguarding services.

Risk To Others

If known or suspected risks of physical harm to others are present, we are unable to accept a referral. Please instead refer to appropriate local social support or safeguarding services.

Survival

If known recent or ongoing significant adverse life events or longstanding difficulties in managing basic activities of daily living, we may not be a suitable service to accept a referral. Please ensure you detail what these presenting factors are so we can make an informed decision about the appropriateness of a referral.

Psychological

If known, complex presentation, severe and enduring mental illness, unmanaged symptoms or persistent disengagement from services is evident, we are unable to accept a referral. Please instead refer to appropriate local mental health services. Please detail disabling or distressing problems with thinking, feeling or behaviour so we can make an informed decision about the appropriateness of a referral.

Social

If known inability to engage with an online service, e.g: due to lack of access to appropriate digital devices or lack of reliable internet access, we are unable to accept a referral. Please instead refer to a more suitable local or non-digital service provider. Please ensure you detail severity of problems with social activities or in relationships with other people so we can make an informed decision about the appropriateness of a referral.

Substance/Alcohol Misuse

If known harmful use of one or more substances (drug or alcohol), enduring addiction with likely or possible treatment resistance we are unable to accept a referral. Please instead refer to appropriate local mental health &/or substance use or addictions services.

If you decide to make a referral, please provide appropriate details regarding presenting risks to self or others, interpersonal communication and relationship difficulties, levels of self-neglect, unsafe behaviours or difficulty maintaining a safe environment and severity of problems with thinking, feeling or behaviour. This will allow us to make an informed decision about the appropriateness of a referral.

What kind of personal data do we collect?

– Basic contact information (name, date of birth, telephone numbers and address).
– Current and past medical history, medication, GP details, treatment records.

How do we use your data?

– We use your contact information to get in touch with you to organise appointments and treatment
recommendations as well as sorting payments.

– We may liaise with other professionals involved in your care, such as your GP, hospital consultant, solicitor, your case manager or other members of the multidisciplinary team.

– We will ask for your consent before we proceed with the above.

How do we store your data?

– Neurorehab Hub stores and processes information in accordance with the General Data Protection Regulation (GDPR).

– Your information is securely stored electronically on our private practice manager software and on Microsoft protected with multifactorial passwords and key fobs.

What are your rights?

Under data protection law, you have rights including:
